How to take the second dose of Mounjaro is one of the most common early questions, and the reassuring answer is that the second dose is identical to the first. Same strength, same day of the week, same technique. The only things that change are the injection site and, if you are using a multi-dose pen, which of the four doses you are on. This is the single most important point, and the one most often misunderstood. How to take the second dose of Mounjaro does not involve stepping up the strength.
Treatment begins at 2.5 mg once weekly and stays there for the first four weeks. Your second, third and fourth injections are all at that same starting strength. Only after four full weeks does your prescriber consider moving to 5 mg.

When your second dose is due
Mounjaro is taken once a week, on the same day each week. Your second dose is due seven days after the first, at any time of day, with or without food.
Picking a consistent day matters more than the hour. Many people mark it on a calendar or use the dose-tracking chart included in the Instructions for Use. If you need to move your injection day, you can, provided at least 72 hours have passed since your last dose. Once you choose a new day, keep to it weekly.
Which pen do you use?
This depends on which presentation you were given, and it is where most of the practical confusion sits.
If you have a KwikPen: each pen contains four fixed doses, one taken weekly. Your second dose comes from the same pen you used the first time. One pen covers roughly a month, and it is discarded after four doses or 30 days after first use, whichever comes first. You will see leftover liquid after the fourth dose; that is normal and must not be used.
If you have single-dose pens: you take a new pen for each injection, including the second.
Either way, you use a new needle every single time. Needles are never reused.
How to take the second dose of Mounjaro step by step
If you are using a KwikPen, the Instructions for Use set out the sequence. Follow them each time rather than from memory:
- Check the pen. Confirm the correct strength, that it is in date, undamaged, and that the liquid is clear and colourless to slightly yellow.
- Attach a new needle and remove the caps.
- Prime as instructed. Turn the dose knob until you hear two clicks and the extended line shows in the dose window, then push the knob in until it stops and hold while counting slowly to five. "0" must appear in the dose window. This goes into the air, never into your body.
- Dial the dose. Turn the knob until it stops and "1" shows in the dose window. That equals one full dose.
- Inject. Insert the needle, push the dose knob until it stops, and hold while counting slowly to five.
- Confirm. Check that "0" is showing before removing the needle. That confirms the full dose was delivered.
One safety point worth memorising: if "0" does not appear, reinsert the needle and complete the injection. Do not redial a new dose. Redialling risks giving yourself more medicine than prescribed.

Choosing a different injection site
Approved sites are the abdomen, the thigh, or the back of the upper arm if someone else is injecting for you.
For your second dose, rotate to a different site. You can stay in the same general area, for example the abdomen, but choose a different spot within it rather than the exact place you used last week. Rotating gives the tissue time to recover and reduces the chance of irritation or lumps building up over months.
What if you missed or mistimed it?
This is the second most common query about how to take the second dose of Mounjaro, and the rule is specific.
If four days or less have passed since the dose was due, take it as soon as you remember, then continue on your normal schedule. If more than four days have passed, skip that dose entirely and take the next one on your usual day.
Never take a double dose to make up for a missed one, and keep at least 72 hours between any two injections. If you have missed several weeks, speak to your prescriber before resuming, as restarting at a lower dose may be appropriate.
If the first dose was difficult
Nausea and digestive upset are most noticeable in the days after starting, and they typically ease as your body adjusts. Smaller, lower-fat meals and good hydration help. If symptoms were severe, or you are worried about repeating them, contact your prescriber before the second dose rather than skipping it quietly. Pace can be adjusted, but only clinically.
